For patients with insurance coverage, please be aware that some procedures are not covered by your plan. These exclusions are listed in your benefits booklet, and you should review these before your appointment.
For patients without insurance coverage, payment is due at the time of treatment. We accept Visa, MasterCard, American Express, and Apple Pay, as well as outside financing through CareCredit. Consideration is given to every patient’s individual circumstance, and we do our best to make treatment affordable.
If you have insurance, amounts due from you are estimated only. Your actual benefits may be less. The insured, for their own information, should verify the benefits available for the patient. The patient or responsible party should also verify any co-pays, deductibles, and remaining benefits for the plan’s year.
Should benefits be obtained, it is not a guarantee of payment. Claims must be submitted and reviewed before any determination is made by the insurance company.
Payment is due for any co-pays, deductibles, or non-covered procedures at the time the procedures are rendered.
As a courtesy to you, we submit all the necessary forms to your insurance company; however, the fee for our professional services are ultimately your responsibility.
Even though you may be covered by dental and/or medical insurance, you are responsible for the balance of any fees not covered by insurance. We will try to delineate before surgery what your out-of-pocket expense will be. You should be aware that most insurance companies pay for only a portion of the costs for services; very few pay the entire fee.
We will bill your insurance for three (3) months after services are rendered. During that time, you will receive statements. If your statement does not reflect your insurance payment within that three-month timeframe, please call our office.
Interest will be accrued to all amounts remaining unpaid after the three-month billing period at the rate of 1.5% per month. If your account should become delinquent, you are required to pay for all remaining charges, interest, collection costs, and attorney fees.
